Idabel Public Schools is partnering with Choctaw Nation Health Services to provide a FREE (reguardless of insurance or income) shot clinic. The TDAP, HPV and Meningococcal vaccines will be provided. Please sign and return the attached permission form before Friday, April 8th, we will be vaccinating that morning only.
Parents do not need to be present but permission slips MUST be signed.
